Requirements:

• Minimum 3.5 GPA
• Resume
• Participation in 1 service events during the academic year 2018-19. 
• Other campus involvement and community service
• Personal statement, 250 words or less, describing service and participation in Phi Eta Sigma 
• Submit application by email to m.harper@utah.edu by Friday, March 1st.
